A Japanese government survey has revealed about 20 percent of child care facilities including kindergartens have issues with their school bus operation.
                
Municipal offices conducted on-site surveys on behalf of the central government from September to December of last year covering about 10,000 facilities across the nation.
                
The move followed the death of a three-year-old girl who was taken to her child care facility by bus in Shizuoka Prefecture, central Japan. On arrival, she was forgotten inside the bus for hours and died of heat stroke.
                
The results of the survey show 19.5 percent of facilities answered that they had problems like not being able to account for all children when they get on and off the bus.
                
At some facilities, employees do not share information about children who are absent without notifications from their families. Some buses had stickers and shaded windows, which makes it difficult to see what is going on inside.
                
The survey has identified a problem with insufficient training of employees to prevent such a tragedy from happening again.
                
A government official says about 90 percent of the facilities with issues say they are scheduled to make improvements by the end of March. The official said the government will closely monitor their response.
